YMO Application Form

This form should be used by a parent/guardian of a young match official (age 14-17) to apply to join the society on their behalf.

All information provided on this form will be used within the guidelines of the Data Protection Act 2018 and the Dorset and Wilts Society of Rugby Football Union Referees Privacy Policy (copy available on request or electronically via the society website). It will only be used for the purpose that it was provided.

Please confirm the following:

We are a forward thinking Society and are actively looking to increase the number of referees available to cover the games throughout the Counties and support new referees to accomplish the highest standard possible at whichever grade they reach.
